LegitScript certification is the first major requirement standing between an addiction treatment center and the ability to advertise on Google and Meta. Without it, ads for detox, residential care, outpatient programs and other recovery services are not allowed to run in the United States, no matter how well the campaign is built. For many programs it is the first hurdle in paid advertising, and one of the most misunderstood.
We have helped treatment programs work through certification and then managed their Google Ads once they were approved, so we have seen what happens on both sides of that approval. This guide covers the requirements and costs, but also what the official documentation leaves out: where applications actually get held up, and why a certified program can still see its ads disapproved.

Why Google and Meta Require LegitScript for Addiction Ads
Addiction treatment is one of the most heavily restricted categories in online advertising. Google’s healthcare and medicines policy says it plainly: to advertise recovery-oriented drug and alcohol addiction services, you must be certified as an addiction services provider by the LegitScript Certification program, and you must also be certified with Google. Meta’s drug and alcohol addiction treatment policy takes the same position for ads that reach people in the United States.
The restrictions exist because the industry has a history of patient brokering, misleading ads and call centers posing as treatment providers. LegitScript acts as the independent check. It reviews who owns the program, whether it is licensed, who provides care and how it markets itself, so the ad platforms can trust that a certified advertiser is a real, legitimate provider.
Google’s addiction services policy applies to recovery-oriented drug and alcohol services, and certified advertisers can promote them in the United States, Canada, Australia, France, Ireland and New Zealand. Services for behavioral addictions, impulse control disorders or nicotine addiction fall outside this policy.
What LegitScript Certification Costs in 2026
LegitScript charges two fees per facility: a nonrefundable application fee when you apply, and an annual certification fee once you are approved. Both drop as the number of facilities grows. These are the current rates from LegitScript’s addiction treatment certification page:

Individual practitioners, such as a counselor in private practice, pay a lower rate: $535 to apply and $1,070 a year. If you need a faster start, LegitScript offers expedited processing for $2,500 per application. That is on top of the regular fees, not instead of them: it gets your review started within two business days of submission, though approval still depends on how complete your application is. For a single location, expediting brings the first-year cost to $7,190 ($1,595 application, $2,500 expedited review and $3,095 annual fee).
Two details catch programs off guard. The application fee is not refunded if you are denied, and the annual fee is not refunded either, although you can cancel your certification in writing at any time. For a single-location program, that is $4,690 in the first year before a single ad runs, which is why it pays to apply only when your paperwork and website are ready.
Who Qualifies for LegitScript Addiction Treatment Certification
LegitScript certifies licensed addiction treatment services in the United States and Canada. That includes detox, residential treatment, outpatient treatment and counseling, and medication-assisted treatment, along with mutual support groups and crisis hotlines that offer addiction-related help.
Just as important is who does not qualify. LegitScript does not certify lead generators, call centers or marketers that refer patients to other providers, and it does not certify sober living homes or recovery residences that lack licensed clinical services. That creates a real catch for recovery housing: Google’s policy lists sober living environments among the services that need certification, but a sober living home without clinical services cannot get certified, so it cannot run addiction-related ads on Google.
The same thinking extends to relationships. Co-owning or having any business affiliation with a lead generator is prohibited, and LegitScript expects your affiliates and partners to meet its standards too. A program that shares ownership with a referral website can be denied even if its own clinical care is excellent.
Does LegitScript Apply to Your Type of Program?
Whether you need certification depends less on your title and more on what you treat and how you advertise. Here is how it breaks down for the four types of programs we work with:

The prescribing point applies across the board. Google treats online prescribing as its own restricted category, and LegitScript covers it through its separate healthcare certification for providers that diagnose, treat and prescribe remotely. A mental health practice that never mentions substance use and never prescribes online does not need LegitScript at all, although its ads still have to follow each platform’s general healthcare rules.
Lead Generators, Referrals and the Federal Law Behind the Rule
LegitScript’s line on lead generators mirrors federal law. The Eliminating Kickbacks in Recovery Act (EKRA), passed in 2018 as part of the SUPPORT Act, makes it a federal crime to pay or receive anything of value for referring patients to a recovery home, clinical treatment facility or laboratory. It covers any health care benefit program, private insurance included, with penalties of up to $200,000 and 10 years in prison per occurrence.
The distinction that matters is what you are paying for. Paying a marketing partner to do the work, such as running your ads, building your website or improving your search visibility, is buying a service. Paying someone for each patient they send you, or a fee that rises with every admission, is paying for a referral, and that is where the law draws the line. It reaches inside your own walls too: the exception for employees and contractors only holds when pay is not tied to referrals or billing, as the full text of the statute spells out.
It is also why call centers that screen callers by insurance and route them to whoever pays most draw so much scrutiny. In one series of federal cases in Orange County, prosecutors alleged facility owners valued patients by their insurance and paid recruiters per referral.
The practice has not disappeared. Programs still receive per-admission offers from lead sellers, some dressed up as marketing. We are not here to police the industry, but it is worth recognizing those offers before one lands in your inbox. The Standards LegitScript Reviews Most Closely
LegitScript measures every application against its certification standards. A few of them decide most outcomes, and they are worth preparing for before you apply:
- Proper licensing and registrations for every facility, with your policies and procedures
- Disclosure of any legal or regulatory history from the past 10 years
- The qualifications of your clinical staff
- A physical address shown clearly on your website
- Ads and website content that are accurate, transparent and not misleading
- Disclosure of any incentives offered to patients
Expect to gather business licenses, clinical policies, ownership information, facility-level licensure, current DEA registrations where they apply, and a list of every domain you own. LegitScript reviews all of your websites, not just the one you plan to advertise, so an old landing page with claims you would no longer make can slow things down.
How Long Does LegitScript Certification Take?
There is no fixed answer, and LegitScript does not publish standard review times. How long your review takes depends heavily on how prepared you are: how complete your application is, how quickly you respond when reviewers ask for more information, and whether your licenses, website and domains all line up the first time. Every round of follow-up questions adds time.
The one timeline LegitScript does commit to is for expedited review. For an extra $2,500 per application, on top of the regular fees, your review starts within two business days of submission. That speeds up when the review begins, not how long it takes to finish, so it pays off only if your application is already complete and consistent.
Build the platform step into your plan as well. LegitScript approval is followed by separate Google and Meta reviews before your ads can run, so the realistic question is not how long certification takes, but how long until your campaigns are live.
Getting Approved Is a Two-Step Process
LegitScript approval is not the finish line. It is what allows the next part of the process to begin. We have worked with treatment providers through certification and then managed their Google Ads afterward, and the most common misconception we hear is that once the approval email arrives, advertising is switched on everywhere. It is not. LegitScript certifies that you are a legitimate provider. Google and Meta each run their own review before your campaigns can run normally.
On Google, once LegitScript has certified you, you apply for healthcare-related advertising from your Google Ads account, selecting addiction services as the certification type. On Meta, you request permission to run addiction treatment ads through Meta’s own form. Until each platform approves you, ads for restricted services will keep getting disapproved, even with a LegitScript certificate in hand, and some accounts are asked for additional verification along the way.
This is also where small inconsistencies cause delays. Your business name, website, treatment locations, licensing and the services you advertise need to tell the same story across LegitScript, your website and your advertising account. Our advice is to prepare for advertising before certification is complete: have the Google Ads account, landing pages, conversion tracking and campaign structure ready to launch, rather than starting to build everything after approval.
Virtual Programs and Telehealth: One Certification or Two?
Programs that offer virtual IOP or telehealth counseling often assume they need an extra certification. In most cases they do not. LegitScript’s addiction treatment certification covers licensed treatment services whether they are delivered in person or remotely, so a virtual outpatient track is covered by the same certification as your in-person program.
The exception is prescribing. If your remote services end in a prescription being issued, such as remote medication management, Google’s telemedicine rules apply, and those are met through LegitScript’s separate healthcare merchant certification. The question to ask is not whether care happens over video, but whether prescriptions are written remotely.
The LegitScript Problems We See Most Often
LegitScript’s published standards name several grounds for denial: recent or repeated disciplinary sanctions or warning letters, prior offenses, and failing to provide full and accurate information when asked. One more is easy to trigger without realizing it: advertising addiction services on platforms that require certification before you are certified can itself be grounds for denial.
In our experience, though, the problems we run into most are rarely dramatic compliance failures. They are mundane digital inconsistencies: an old landing page still live for a service the program no longer offers, a second domain nobody listed, a new service added to the website before the certification was updated, or a location name that reads one way on the website and another on the license. None of it reflects on the quality of care, but each one is a mismatch a reviewer can see. LegitScript and the ad platforms look at your entire digital footprint, not just your homepage, so every one of those gaps shows.
Before a client applies, we review the website the way a reviewer might:
- Does it clearly explain who provides treatment and where it is provided?
- Are your levels of care, licenses and accreditations easy to find and current?
- Are there outdated pages that contradict the program you run today?
- Are other domains or landing pages in use that need to be listed?
Staying Certified and Keeping Your Ads Running
Certification is not a one-time event. LegitScript monitors certified providers on an ongoing basis to confirm they still meet its standards and the ad platforms’ policies, and the annual fee keeps your certification active. Keep LegitScript updated as your organization changes. Adding detox, opening another location, changing a level of care or launching a new treatment-related domain can feel like a marketing decision, but each one can affect your certification and your advertising compliance.
Certification also does not protect you from disapprovals, which surprises many treatment centers. It establishes that you meet LegitScript’s requirements. It does not give you permission to say anything you want in an ad. The kinds of claims that cause trouble are predictable: success rates or outcomes you cannot substantiate, language that implies guaranteed recovery, insurance wording that sounds broader than the plans you actually accept, and landing pages advertising a level of care that does not exactly match your license or certification. Any of these can get ads or landing pages disapproved, even for a fully certified advertiser. We have also seen reviews get harder when an ad sends people to a page that describes your services differently from your main website.
Getting certified gets you through one gate. Your ads and landing pages still have to pass the next one.

Most programs pursue LegitScript because the ad platforms require it. The certification does more than unlock ads, though. It is one of the few independent signals in this industry that tells a stranger your program has been checked by someone other than you.
That matters to the people deciding where to call. Families and patients researching treatment have usually heard about bad actors, from misleading call centers to programs that are not what they claim to be. Certified programs can display the LegitScript certification seal on their website, and because the seal links to a live status page on LegitScript’s own site, anyone can click it and confirm the certification is real and current. Anyone can also look a program up with LegitScript’s website certification status tool.

In LegitScript’s words, displaying the seal gives visitors confidence that your business meets LegitScript’s rigorous standards. It is a credential that a program cannot award itself, which is exactly what makes it persuasive.
That makes the seal a meaningful third-party trust signal, alongside your state licensing and accreditation. Placed where people make their decision, such as near your admissions phone number, on your insurance verification page and in your footer, it answers the question many callers are quietly asking: can I trust this place? On a site built to turn visitors into calls, removing that doubt at the right moment is part of how a conversion-focused website earns its inquiries.
A few practical notes. The seal is optional, and leaving it off does not affect your certification or your ability to advertise, but it only builds trust with visitors if you actually add it, and LegitScript requires its official code rather than a copied image, which is what keeps the verification link live. The same credibility carries beyond your website: referral partners, business contacts and anyone checking your program’s reputation can confirm your standing in seconds, and the certification process itself tends to leave you with licensing, policies and marketing claims that are organized, current and ready for scrutiny.
Turning Certification Into Admissions
For a treatment center that wants to use paid search, certification changes the whole conversation. Before it, everything is about restrictions. Once certification and the platform approvals are in place, the focus can move to real marketing strategy: which programs to promote, which markets to target, which searches bring in qualified calls, how your landing pages convert and what it costs to generate an admission.
Certification does not make a campaign successful on its own. We have seen certified advertisers with poorly structured campaigns, weak landing pages, loose geographic targeting and a lot of wasted spend. Certification gives a qualified provider access to the channel.
